The Health Information Management (HIM) staff at Smith County Memorial Hospital is committed to respecting and protecting the rights of our patients and families. The HIM staff is responsible for maintaining current medical records on all of the patients of the hospital. These records are accessible to the physicians 24 hours a day.

It is sometimes a common misconception that the hospital have a record of birth certificates, however, all Birth Certificates are filed with the Kansas State Office of Vital Statistics and are not kept at the hospital.
